Online nomination for Singapore Youth Award
All nominations must be submitted online by 06:00pm (GMT+8), 15 January 2010.
Please ensure that the nomination form is completed in full. Incomplete nomination forms will not be considered.
All soft copies of the supporting documents should be uploaded together with the nomination form. Nominees are advised not to submit original copies of the supporting documents as the Award Secretariat will not be held responsible for materials lost in transit. All materials submitted will become the property of the Award Secretariat and will not be returned.
To protect their online identity, nominees are advised to keep their user ID and password strictly confidential. Any confidential information submitted should be clearly labelled as such. The Award Secretariat reserves the right to disclose the nominees' non-confidential information as it deems appropriate for the purpose of the Award.
