Nominating someone
You would need to furnish some basic information about yourself and your nominee. A notification email would then be sent to the nominee to complete the nomination form.
Registration
Before completing the nomination form, it is necessary for you to register an account so that you can log on to the system. To register, you would need to click "Create an account to start nomination" at the nomination main page and provide your personal particulars. Your NRIC would be used as your user ID.
Note: If you have been registered as a nominee previously, you do not need to create an account again. Simply use your existing login details to complete and submit your nomination.
What should I do if I have forgotten my login password?
You would be required to contact the SYA Secretariat at Tel: 68399124 / Email: enquiries@sya.sg to retrieve your password. Please provide your NRIC number for retrieval purposes.
What should I do if I have forgotten my user ID?
Your user ID is your NRIC number.
What happens if I have more than one nominator?
You only need to indicate one nominator in the form. However, please note that your nomination would be considered for the Award only if it is duly endorsed by your nominator after submission.
Must all fields in the nomination form be completed?
Yes, all fields must be completed. For fields that do not apply to you, please indicate them with "N. A".
Interruption of data entry
You can interrupt your data entry session at any time after you have 'saved' the completed screen. When you log on to the system next time, you would be able to continue data entry for the partially completed nomination form.
Note: Once your nomination has been submitted, you would no longer be able to access or make any modifications to the form.
Nomination main page screen
This screen is the home page for your nomination. You are also able to print and save a copy of your completed nomination form.
Nominator details screen
This screen requests information about your nominator, including his / her organisation and contact details.
Nominee details screen
This screen requests information about the nominee, including his / her organisation and contact details.
Uploading documents
You have to attach all the required supporting documents for your nomination. They can be removed or replaced prior to final submission.
Note: Files must be in Rich Text (.rtf), Adobe Acrobat (.pdf), Microsoft Word (.doc), Microsoft Excel (.xls) or Microsoft Powerpoint (.ppt) format.
Submission of nomination form
All sections have to be completed before you can submit your nomination form. To submit, click the "Submit Application" button located on the online nomination form. Upon submission, the form will be locked and a system-generated email would be sent to your nominator for endorsement.
Note: Once your nomination has been submitted, you would no longer be able to access or make any modifications to the form.
Confirmation of successful submission
Your nomination is considered as a successful submission only after it has been duly endorsed by your nominator. Please note that all forms have to be endorsed before the stipulated closing date of the SYA nominations. |
